I would take @Jason Stoddard ’s voltage comment seriously. Voltage surges do occur and they wipe out many components.
Could it be the problem at the wallwart? The MM2 never turns off the rear power switch since connected.

Also, are you using the same AC wallwart in the new MM as in the old one? Ie. you just swapped MMs, and used the same wall wart? It might be worth swapping wall warts too if that's the case because that could be causing the issue.
No, I use the new one comes with the 2nd unit.

Speaking of Folkvangr (contemplating pulling the trigger on a classified), how in the world does one know when a power tube is going bad, and which one is the culprit?
FV can work with 2-8 Power tubes. It might start having noise if tubes go bad.

I have a black FV with a set of best tubes for it for sale if you're interested in :)

This is the one failed last Feb.
edited (after reading post about using 2nd wall wart):
For the replacement unit, request a replacement wall wart too. discard your existing one (unless schiit wants is back for testing / failure analysis). wall wart may have intermittent short between primary and secondary windings, solder short, insulation failure,

@Ohno Schiitizen make sure that USB power adapter connected to the apple CCK is of old-school 5V only type and NOT one of the higher power (e.g. 20W) rapid charger USB-C types that has variable-voltage output.

Owning a tube amp means building in time for future issue-hunting and diagnosing, and in this case rotating each power tube out until something changes.
